Credit Application

Ready to start or expand your account with Crystal Valley?

Forms can be filled out, saved, signed, and emailed or mailed to our credit department for processing (typically within 5 business days).

About Application Process

  • 1

    Account applications may take up to 5 business days from the time they are received by the Crystal Valley credit department to be reviewed and processed.

  • 2

    The federal government requires all accounts to complete the W‑9 form. The name that is provided on the W‑9 must match the name on the account application.

  • 3

    The Sales Tax Exemption certificate (Certificate of Exemption, Form ST3) is to be completed by accounts exempt from state sales tax.

Important: Incomplete applications may cause delays in review and account setup.

All forms may be filled out electronically, printed, signed, and emailed to creditdepartment@crystalvalley.coop or mailed to the information listed on the application.

Applicants who indicate the following bank references need to complete an additional step.

  • Home heat only

    Does not need to complete this step.

  • US Bank and Wells Fargo customers

Credit Policy

Our credit policy outlines eligibility, payment terms, and account guidelines. It includes payment deadlines, late fees, and how patron equity affects your financial standing with Crystal Valley.

Payment Notice

All payments presented to Crystal Valley accept the following terms:

  • All payments returned to Crystal Valley as UNPAID will be assessed a $30 fee.

  • Maximum state allowable service charge will be added.

  • Civil penalties may be imposed for NON-payment after 30 days.

  • Crystal Valley reserves the right to Electronic Funds Transfer (EFT) or demand draft your account for the amount of the payment and/or fees.

  • Crystal Valley will add a 3% convenience fee on the transaction amount when paying with a credit card. There is no fee for debit card payments.
